Submit payment disputes and Independent Dispute Resolution (IDR) requests
If you wish to initiate a request for negotiation during the 30-day open negotiation period for purposes of disputing the total amount of payment or a specific Qualified Payment Amount (QPA), please submit your request and all relevant claim documentation using this form.
Generally, an Out-of-Network provider or facility may initiate the Federal IDR process within 4 business days after the end of the open negotiation period. Submit your IDR requests and determinations using this form.
